About direction

FeNO is the level of nitric oxide (NO) in the air you breathe out. Cells in your airways make it when allergic inflammation is going on. The higher the number, the more active the inflammation. The test helps confirm asthma, including hidden asthma, and shows whether treatment works. At Oberig Clinic we do it on the Niox Vero device for adults and children. The result is ready in a few minutes.

Who needs it

Most often an allergist or a lung doctor orders it. Typical reasons: a long cough without a cold, wheezing, shortness of breath on exertion, coughing at night or after dust, pets or pollen. FeNO also helps decide whether inhaled steroids will work. Once bronchial asthma is diagnosed, it shows how well they act.

How the test goes

You sit down, put a single-use filter in your mouth and breathe out slowly and evenly into the device for about 10 seconds. The screen shows you how hard to blow. Kids have a 6-second mode. That is all. No needles, no discomfort. The whole visit takes 5-10 minutes, explanation included.

For one hour before the test, do not eat, drink anything but water, smoke or exercise. If you also have spirometry that day, FeNO goes first. Tell the doctor which inhalers you use.

Normal range and what the result means

The result is measured in ppb (parts per billion). International guidelines give rough cut-offs. In adults, under 25 ppb means inflammation is unlikely, 25-50 ppb is a gray zone, and over 50 ppb means allergic inflammation is likely. For children the cut-offs are lower: under 20, 20-35 and over 35 ppb. The doctor reads the number together with your symptoms and spirometry, never on its own.

Some things shift the number. Smoking lowers it. A fresh viral infection or contact with an allergen the day before raises it. Inhaled steroids calm the inflammation and bring it down. So repeat tests work best under the same conditions.

Common questions

Is FeNO a blood test?

No. Nobody takes blood. The device only measures the air you breathe out. That makes it a good fit for kids who fear needles.

From what age?

Usually from 4-5 years, once a child can breathe out evenly for a few seconds.

Does it replace spirometry?

No. Spirometry shows how your airways work. FeNO shows whether they are inflamed. Together they give the full picture.
